It seems to review each comment and compare it against a remote service which collects reports from blogs which have been spammed.  It has multiple other factors as well so that "clean" comments appear immediately while moderately suspect comments are held for review.  And you can throttle the behavior.  I believe when you do mark a comment as spam it submits that information back to the Spam Karma system to help out everyone else.  No need for Captcha or tricky Javascript which can be compromised.</description>
